Holmes County Chronic Disease Program is a grant funded program focused on providing education and offering assistance in making policy and environmental changes within Holmes County to encourage a healthier lifestyle.  The Chronic Disease Program works to address various aspects of health, from nutrition and physical activity to diabetes and tobacco cessation.

The Florida Department of Health in Holmes County had the opportunity to host Tosca Reno, best-selling author of the Eat-Clean Diet.  Tosca partnered with the Florida Department of Health and Girl's Night Out to provide resources that can help you formulate your own fitness program and meal plan.
