DOH-Holmes Primary Care Program
Our Primary Care Program is designed to serve individuals and families with financial needs as well as those that are self-pay or have insurance coverage.
We provide a medical home, offering:
- Acute health care for illnesses
- Chronic health care
- Preventative health care
